Video Request Form
 

Please fill out the following form to request tapes via campus mail. Make sure all parts of the form are filled out completely so that I can get the correct tape to you at the right time for your class. I will send you a confirmation of your request via e-mail.

Please allow at least one week advance notice so that if the tape has already been reserved by another user, other arrangements can be made. Tapes are sent out on Wednesdays in campus mail, or may be picked up in the library between 8am and 5pm, Monday through Friday. If you have any questions, please call Catherine Wilson at 328-6022.
